Your health information security is our top priority. Please be assured that all patient charts and medical records remain strictly confidential, secure, and encrypted in compliance with health information privacy regulations.

To maintain continuity of care and ensure your records are properly authorized under new ownership, all returning patients must sign an updated Patient Consent & Authorization Form prior to receiving ongoing treatment or accessing complete chart details. You will receive a prompt to complete this securely online prior to your visit.

**Direct Billing & ICBC **

We offer direct billing to most major extended health insurance companies. To set up your direct billing, please bring your insurance card to your next appointment or securely upload your insurance details directly into your Jane App account profile before your visit. We directly bill to ICBC Patients need to provide a claim number before treatment

24 Hour Cancellation Policy:

• Please give us at least 24 HOURS NOTICE to reschedule or cancel your appointment
• Patients who provide less than 24 hours notice, or miss their appointment (no show), will be charged the FULL appointment fee. This fee CANNOT to billed through insurance providers or ICBC
• Frequent the cancellations and no-shows will result in restricting being made to the patient’s ability to book for future appointments in our clinic
